The issue with the map is definitely with your API key. We entered one of our APIs and it is working perfectly. So please make sure you are generating the key in the right way.

You can use WPML plugin for multi language purposes. The theme is fully compatible with it.

Extending the functionality for all fields is more complex and takes a lot of time. We suggest you to use the following plugin to build your forms the way you want.

Please note before using the above plugin make sure to delete the related custom code (both CSS and JS) added from us.

Looks that the issue is caused by the 3rd party plugin. The plugin you're using does not support multi level dependency. We advise you to contact plugin author and seek help from him.
